Select the referral pathway that applies to you. Each pathway has specific eligibility criteria — please read the requirements before proceeding.

Pathway 1

GP referral

For General Practitioners referring a patient who holds a valid Health Care Card and meets the clinical threshold for specialist psychiatry.

Commissioned service referral

For HNCPHN-commissioned mental health services referring a patient in your care into the Telehealth Psychiatry program.

IAR-DST Level 3 or 4 GP referral needed HCC or financial distress

Pathway 3

ACCHO referral

For Aboriginal Community Controlled Health Organisations within the HNCPHN catchment area referring a patient who holds a Health Care Card or is experiencing financial distress.

Check patient catchment eligibility
All pathways require the patient to reside within the HNCPHN catchment area (Port Macquarie to Tweed Heads).

Not sure which pathway applies? Contact the Dokotela intake team before submitting. Internal intake will decline any referral where the incorrect pathway has been selected or where eligibility criteria are not met. All pathways require the patient to reside within the HNCPHN catchment area, be aged 12 years or older, and be referred for treatment and management only (not medico-legal reports or standalone assessments).
